This is a modification of Roux-en-Y Gastric Bypass. The main difference consists that in the intestinal reconfiguration a single anastomosis is performed. This procedure consists of the creation of a gastric pouch of between 70 and 100 mL, and the creation of a biliopancreatic limb of between 150 and 200 cm of the proximal small bowel.

Since in this procedure, the percentage of excluded small bowel is greater than in the RYGB, a greater weight loss will be recorded. However, this could lead to a greater risk of malnutrition and vitamin deficiencies.

On the other hand, its main advantage is related to its easier nature, compared to RYGB, and its lower risk of surgical complications. Therefore, due to the latter, and the consequent greater weight loss, this procedure is an excellent option for patients with a BMI > 50 kg/m2 (super-obese).

This procedure is not recommended for patients with a BMI <45 kg/m2.
